Role Overview

The Patient Care Coordinator is a pivotal role focused on actively managing Patient Support Programs to provide timely and compassionate assistance to patients. The coordinator serves as both an advocate and a guide, ensuring that no patient feels unsupported or lost during their treatment journey. This role is instrumental in navigating the complexities of healthcare systems and aligning patient needs with available resources to maximize their treatment experience.

Key Responsibilities

• Collaborate closely with line management and assigned program teams to efficiently manage Patient Support Programs.
• Maintain detailed and accurate patient records and ensure submissions are completed as per project standards.
• Facilitate the enrollment process for eligible patients according to program protocols and timelines.
• Educate and coordinate with patients to ensure smooth transitions and adherence to treatment plans.
• Visit healthcare providers, pharmacies, and patient sites alongside team members to ensure effective program implementation.
• Guarantee timely delivery of medications to patients, addressing any logistical concerns.
• Generate and prepare reports reflecting program performance metrics, patient numbers, and service quality in collaboration with the team.
• Regularly update and manage patient data within electronic systems while adhering to quality assurance standards.
• Participate in all mandatory trainings focusing on pharmacovigilance and product initiatives as required.
• Promptly report any adverse events or product complaints following company policies and protocols.
• Carry out administrative duties associated with program operations and provide general office support.
• Uphold compliance with international standards, AstraZeneca's Code of Conduct, and internal SOPs.
• Maintain proactive communication with related divisions and external partners, ensuring alignment in program objectives.

Requirements

• A Degree or Diploma in any medical field or social work.
• 0-1 year of experience in healthcare, scientific field, or NGO.
• Experience in data monitoring and management is a plus.
• Experience in logistics/supply chain coordination is an advantage.
• Background in customer service management is beneficial.
